Registration fee is $150.00 per band, payable to Prairie State College, and should be sent seperately. An invoice will be presented upon submission of this form.

Music stands and piano provided. Plan to bring your own drums, amplifiers, and auxiliary
percussion instruments. Deadline for registration is January 22, 2010.
